How Long Does a Hip Replacement Last?

Modern hip replacement is designed to relieve pain and help patients return to the activities they enjoy.

One of the most common questions I hear from patients is:


"How long will my hip replacement last?"


For many years, patients were told that a hip replacement would last approximately 15 to 20 years. While that estimate was reasonable based on older implants and materials, modern evidence suggests today's hip replacements may last substantially longer.


The truth is that the answer depends on which generation of implants we're talking about.


What Did Older Studies Show?


A landmark study published in The Lancet in 2019 reviewed data from national joint registries and found that approximately 58% of hip replacements were still functioning at 25 years without requiring revision surgery.


This study remains important because it provides one of the best estimates of long-term implant survivorship. However, there is a major limitation: many of the implants included were performed decades ago using older materials and implant designs that are no longer commonly used today.


In other words, these results tell us how older hip replacements performed—not necessarily how modern hip replacements will perform.


Why Modern Hip Replacements May Last Much Longer


One of the most significant advances in hip replacement has been the development of highly cross-linked polyethylene (HXLPE).


Polyethylene is the plastic liner that serves as the bearing surface within many hip replacements. Older polyethylene liners gradually wore down over time, producing microscopic particles that could lead to bone loss and implant loosening.


Modern highly cross-linked polyethylene has dramatically reduced wear.


Multiple long-term studies have demonstrated extremely low wear rates, reduced osteolysis, and excellent implant survivorship beyond 15 to 20 years.

When combined with modern ceramic femoral heads, these bearings create one of the most durable combinations currently available in joint replacement surgery.

What Does the Newest Data Show?


A large international analysis published in 2026 evaluated nearly 1.9 million hip replacements performed using contemporary implants and bearing surfaces.


The findings were impressive:

  • 93.6% survivorship at 20 years

  • 92.8% estimated survivorship at 25 years

  • 92.1% estimated survivorship at 30 years


These results are dramatically better than those reported in older studies and reflect improvements in implant materials, implant design, surgical technique, and patient care.


While no implant lasts forever, these findings suggest that many patients receiving a modern hip replacement today may reasonably expect their implant to function for several decades.


Does Activity Wear Out a Hip Replacement?


Historically, surgeons often advised patients to avoid certain activities out of concern that increased activity would accelerate wear and lead to earlier failure.

Much of that concern was based on older polyethylene technology.


Modern highly cross-linked polyethylene has demonstrated remarkably low wear rates, even in younger and more active patients. Long-term studies continue to show excellent survivorship with modern ceramic-on-polyethylene and metal-on-polyethylene bearing surfaces.


This does not mean there is no limit to what an implant can tolerate. High-impact activities place greater forces across the joint than walking, cycling, swimming, or golf.


However, current evidence does not support the common belief that normal recreational activities automatically "wear out" a modern hip replacement.


What Factors Influence How Long a Hip Replacement Lasts?


Several factors still play an important role in implant longevity:

  • Age at the time of surgery

  • Bone quality

  • Body weight

  • Medical conditions

  • Implant positioning

  • Implant design

  • Infection risk

  • Trauma or fractures involving the implant


A patient who undergoes hip replacement at age 50 will naturally place more lifetime cycles across the implant than someone who undergoes surgery at age 80.


For this reason, younger patients have a greater lifetime risk of eventually requiring revision surgery, even when the implant performs extremely well.


The Bottom Line


The commonly repeated statement that a hip replacement lasts "15 to 20 years" is becoming increasingly outdated.


Older studies showed approximately 58% survivorship at 25 years, but those results largely reflect implants and materials from previous generations.


Modern hip replacements utilizing highly cross-linked polyethylene, ceramic bearing surfaces, and contemporary implant designs are demonstrating survivorship exceeding 90% at 20 to 30 years in recent analyses.


While no surgeon can guarantee how long any individual implant will last, today's hip replacements are lasting longer than ever before—and for many patients, may provide decades of pain relief and improved function.
